When to Start Coordinating Transportation
As soon as a discharge date comes into view, even tentatively, it’s worth reaching out. This is especially true for patients who need wheelchair, bariatric, or other specialized transport, since the right vehicle needs to be confirmed in advance.
Information Allegiance Needs
Having the following ready speeds up confirmation:
- Anticipated discharge date and time
- Pickup location and destination (home, SNF, rehab, etc.)
- Mobility level and any equipment needed
- Payer or authorization information, if the trip is Medicaid-covered
